This is the master ledger for the VER standards track. Every known defect in VER 1.0 — from the fifteen issues carried in the Studio PRD's Appendix D to the 189 findings of the August 2026 standards audit — appears here exactly once, with its classification, where it was resolved, and whether it is closed.

§10 adds the 23 August 2026 owner review. Its twenty-eight REV-nn rows are dispositions, not new audit findings: several of them verify an existing finding at source and supersede its earlier documented — not patched disposition, and each such row names the finding it supersedes. The audit's 189 findings stay in their own tables and are still counted exactly once. §10.3 continues the same numbering for findings raised after that review, which are therefore not dispositions of it.

Nothing in this file is normative. The normative corrections live in `standards/ver/1.0.1/VER-1.0.1-errata-annex.md`; the additive proposals live in `standards/ver/1.1-draft/VER-1.1-draft.md`. This ledger says where each item went and why.
